DISCOVERY CALL TERMS & CONDITIONS
The Healthy HabitZ Discovery Call is a free introductory call designed to help us understand your current situation, explore whether support may be appropriate, and outline possible next steps. Completion of the Readiness & Symptom Snapshot questionnaire is required before the call takes place.
The Discovery Call is an information and suitability call only. It is not a medical appointment, diagnostic consultation, emergency service, or replacement for care from your GP, consultant, or other regulated healthcare provider.
2. NO DIAGNOSIS OR TREATMENT
No diagnosis, prescription, treatment plan, or individual medical advice will be provided during the Discovery Call. Any suggestions discussed are general in nature and intended to help you understand possible support options.
You are responsible for providing accurate and honest information to the best of your knowledge. If you have urgent symptoms or concerns about your health, you should contact your GP, NHS 111, or emergency services as appropriate.
4. BOOKING, QUESTIONNAIRE AND ATTENDANCE
Discovery Calls must be booked in advance. Before attending the call, clients are required to complete the Readiness & Symptom Snapshot questionnaire. This forms part of the intake process and helps Healthy HabitZ understand the client’s current symptoms, concerns, and support needs ahead of the call. If the questionnaire has not been completed, the Discovery Call may need to be rescheduled until this has been received. If you need to rearrange your call, please do so as early as possible using the booking link or by contacting Healthy HabitZ directly.
Discovery Calls are usually delivered online. You are responsible for ensuring you have a suitable internet connection, device, and private space for the call.
6. CONFIDENTIALITY AND PRIVACY
Any personal information you provide will be handled in line with the Healthy HabitZ Privacy Notice. This may include information you share about your symptoms, health concerns, wellbeing, and goals.
Healthy HabitZ may collect and process your personal data in order to arrange, deliver, and follow up your Discovery Call. Health information is treated with additional care as sensitive personal data. Further details are set out in the Privacy Notice.
You will only receive marketing emails, newsletters, or promotional updates where you have actively chosen to opt in. You can withdraw that consent at any time.
9. NEXT STEPS AND PAID SERVICES
If, following the call, you choose to book a paid service such as the Menopause MOT, separate terms and conditions will apply.
By booking a Discovery Call, you confirm that you have read these Terms & Conditions and the Privacy Notice, and consent to Healthy HabitZ using the information you provide for the purpose of administering and delivering the call.
