MENOPAUSE MOT TERMS & CONDITIONS
The Menopause MOT is a paid assessment service designed to explore symptoms, health history, lifestyle factors, and possible next steps for menopause and midlife wellbeing support.
The Menopause MOT is an assessment and educational wellbeing service. It is intended to provide a structured review of current symptoms and health history, and may include discussion of lifestyle, nutrition, testing, and support options.
2. NOT A SUBSTITUTE FOR MEDICAL CARE
The Menopause MOT does not replace your GP, specialist, emergency care, or any necessary medical investigations. Healthy HabitZ does not provide emergency services. Clients with urgent or concerning symptoms should seek prompt medical advice.
3. NO GUARANTEE OF OUTCOMES
While every effort is made to provide high-quality, personalised support, no specific outcome or result can be guaranteed.
Clients agree to provide complete and accurate information to the best of their knowledge, including relevant health history, symptoms, medication, supplements, and other information that may affect the assessment.
Where dry blood spot or other functional testing is included, clients understand that testing is intended to provide additional information to support the assessment process, results must be interpreted in context, test results do not replace medical diagnosis, and samples must be taken according to the provider’s instructions.
6. APPOINTMENTS AND FORMS
Any forms or questionnaires requested as part of the Menopause MOT should be completed honestly and submitted before the appointment where possible. Delays in submitting forms may affect the timing of the assessment.
Payment is required in accordance with the booking process in place at the time of purchase. A booking is not confirmed until payment has been received.
8. CANCELLATION AND RESCHEDULING
If a client needs to rearrange their Menopause MOT, they should provide as much notice as possible. Any cancellation or rescheduling policy should be clearly stated at the point of booking.
9. CONFIDENTIALITY AND PRIVACY
Personal and health information will be handled confidentially and in line with the Healthy HabitZ Privacy Notice. This may include questionnaire responses, clinical-style history, test information, reports, and correspondence.
Healthy HabitZ may collect, store, and process personal data and health-related information for the purpose of administering and delivering the Menopause MOT, preparing reports, arranging tests, storing completed forms, and communicating relevant service information.
Information will not be shared with third parties except where necessary to deliver the service, comply with legal obligations, protect vital interests in an emergency, or where permission has been given.
Marketing communications are separate from service communications. Clients will only receive marketing emails where they have actively opted in, and they may withdraw consent at any time.
13. PRACTITIONER DISCRETION
Healthy HabitZ reserves the right to decide whether the Menopause MOT is appropriate for a client’s circumstances and may recommend that they seek GP or specialist input where needed.
By purchasing and attending the Menopause MOT, the client confirms that they have read and understood these Terms & Conditions and the Privacy Notice, and consent to Healthy HabitZ processing the information they provide for the purpose of delivering the service.
